Outdoor / Indoor Affiliate Fees
Returning Youth - received on or before July 1 - $100.00
Returning Adult - received on or before July 1 - $100.00
New Youth or Adult - $100.00 - new members will not be subject to a late fee
Combined Youth and Adult member - received on or before July 1 - $100.00

Medical/Accident Insurance - Youth
(Excess coverage providing secondary insurance to supplement individual’s personal primary coverage)
Cost: Included in player fees $1.80 medical $ .85 liability
Accident-Medical Coverage: $250.00 deductible $ 50,000.00 limit per occurrence
General Liability Coverage: $1,000,000.00 per occurrence. If your affiliate has its own liability insurance and wishes to waive the $.85 per player cost of Georgia Soccer youth liability coverage, you will need to provide a certificate of liability insurance coverage of $1,000,000.00 or higher per occurrence. If no proof of liability insurance is provided, fee per player will automatically apply.

Medical/Accident Insurance - Adult
Cost: Included in player fees
Accident-Medical Coverage: Covers medical/dental expenses incurred within 52 weeks of date of original accident. $400.00 deductible $5,000.00 maximum benefit
Legally licensed treatment must begin 60 days from date of accident.

Liability Insurance - Adult
Cost: Included in player fees.
Coverage: Leagues, teams, coaches, sponsors, directors/officers, and officials/volunteers while acting on behalf of named insured. Additional interest of playing facilities may be added.
Benefits: $1,000,000.00

Cost: Included in the organization affiliation and player registration fees.
Coverage: $1,000,000.00 liability $5,000.00 deductible per loss

Youth Non-Attendance at Annual General Meeting

Affiliates with fewer than 500 players - One (1) vote $100.00 fine
Affiliates with 500 to 999 players - Two (2) votes $200.00 fine
Affiliates with 1,000 to 1,499 players - Three (3) votes $300.00 fine
Affiliates with 1,500 or more players - Four (4) votes $400.00 fine

Affiliation Application

In order to complete your affiliation application, you will need the following:

1. An electronic copy of your organization’s current bylaws; a Certificate of Liability for $1,000,000 naming Georgia Soccer as the certificate holder if, your organization will not be accepting the liability coverage offered by Georgia Soccer. These files / documents can be uploaded in pdf or microsoft word format.doc.
2. Names and contact information for your organization’s officers, especially the required positions.
Required Positions, adults: President/Presiding Officer, Certified Referee Assignor, Registrar, Discipline & Protest Chair, Risk Management Coordinator and Treasurer.
Required Positions, youth: President/Presiding Officer, Treasurer/Financial Officer, Risk Management Coordinator, Certified Referee Assignor, Rec / Select registrars.
3. Contact information for a secondary administrator who will also have access to your organization’s affiliation application
4. Visa, MasterCard, Discover Card, American Express, or echeck for online payment. Payment must be online, not mailed in.
5. The name and street address of all venues /fields used by your organization.
