To request any assessment, send an email to the State Director of Assessments (SDA) with the following information:

• Your name
• Your current grade 
• Purpose of the assessment (developmental, maintenance, or upgrade) -
• Date of the match
• Time of the match
• Location & field number
• Level of play
• Your phone number

Provide at least ten (10) days notice before the assessment date.
